I make a reaction roll just to get the initial attitude the npc/monster has prior to the encounter with the PCs then it is affected by words and actions by the PCs.

In some old games I used fallen bodies as a movement adjustment such as cost additional 5 ft to move through the space. I treat various terrain the same way. Many people take the odt files and make themselves their own compilation then print them out. I heard some tried out pod services to print out a book and others just print out on their home printers then hole punch and put them into a binder.
