Does Assume Animal Form needs a little more clarification?

If slain, the caster reverts back to his or her original form.
Does this means the Druid is actually dead or the spells gives him a free extra life?
The caster gains the physical capabilities and statistics of the new form but retains his or her own mental abilities.
This includes rolling for hit points? A 9th Level Druid keeps his HP when transformed into a 1HD creature or must roll and use that value? If yes, how is the damage transferred between forms? Unless the Druid uses a secondary pool of HD almost earning an extra life as above...

On the first question, it means the caster's dead body reverts to its original form, not that they are revived or anything.

For the second question, that is GM's interpretation. The GM should interpret it how it makes sense to them and communicate it to the player.

Personally, I'd give the caster the creatures STR, DEX, and CON, leaving the rest alone. I also would NOT adjust their HP in any way, even with an increased CON. I'd allow the bonus for saves and such, but their HP is their HP. But that is all just me. Do what makes sense to you.

The hp remains the same.
I use the HD of the creature.
You don't really turn into a cat to fight a bear.
You do it to track, see in the dark, move silently, climb, go through a small crack, etc...
