Index Card Character Sheet

This is a character sheet meant to be printed, cut out, and glued onto a standard 3-inch by 5-inch index card. Printing directly onto index cards works as well, as long as your printer supports it. The sheet is inspired by the 0-level character sheet in the Level-0 Character Funnel but it was recreated from scratch for this supplement.

The print version comes in an individual PNG image as well as a PDF with a set of 4 to be printed on standard US Letter paper. The fillable version is a single card, and, of course, can be filled electronically. I used the free version of Master PDF Editor to add the forms, so opening the PDFs with that program will allow you to edit the forms easily.

Alright, it used Soutane for the form field font on my desktop. Looking at it on my phone though, it defaults to some generic sans-serif. I'm at a loss then. Fortunately, it's mostly meant for printing and then handwriting. I'm not gonna worry about it.

Thanks for sharing this! Funny thing is I was writing characters up on index cards just last night. Great work!

By request, I've been working on creating a back side for the sheet. For now, there is a sheet for casters and one for clerics. The goal is that the Caster sheet should be used by any class that can cast spells, including supplemental classes. Clerics (and Paladins, Druids, and any other class with an ability based off Turn Undead) should generally also use the caster sheet, except in a solo game, since a player should ordinarily not be thinking about their ability's table.

I am also working on a sheet for skill-based classes, like the thief or ranger, as well as improving the front side of the sheet.

r5 is mostly up now. There's a Caster, Clerical, Skills, and a plain Notes version for the back. I managed a little bit of wiggle room on the front to make more space for the Attack field and Carried Possessions. In Wealth, I added fields for denominations of coins, gems and jewelry, and an "other" category. All I have left to do is make a full sheet with four cards on it for each style.
